OUSF Notifications

Notifications of Request, Notifications of Determinations and Requests for Reconsideration for OUSF Reimbursement can be found on the Public Utility Division's Active Cause Report.

To Search by Beneficiary (If not Primary):

To find a notification, click on the T-OUSF-Active tab within the Active Cause Report.  This report can be filtered by clicking on the drop-down box located in the Heading cell of the Beneficiary (if not Primary).  First, remove the check mark for "Select All," then scroll to find the school, library, or healthcare facility you are seeking.  Click the box next to the beneficiary you are seeking and click "OK."  Once the list is filtered, the cause number can be found in the cause number column.

To Search by Applicant / Carrier:

To find a Notification for a specific applicant or carrier, click on the T-OUSF-Active tab within the Active Cause Report.  The report can be filtered by clicking on the drop-down box located in the Heading cell of the Applicant / Carrier.  First, remove the check mark for "Select All," then scroll to find the Applicant / Carrier you are seeking.  Click the box next to the Applicant you are seeking and click "OK." Once the list is filtered, the Cause Number can be found in the cause number column.

To Search by Application Filed Date or Date Range:

Please note that the most recent requests are located at the bottom of the Active Cause Report. If you are seeking to find Notifications for specific dates or a date range, you may also filter the report by date range or specific date by clicking on the drop-down box located in the Heading cell of the Application Filed Date. You may also scroll through the report or sort the report from oldest to most recent or visa versa. Once you've found the date filed, the Cause Number can be found in the cause number column.

To access the Requests For Funding, Determinations or Requests for Reconsideration as filed:

  1. Click here to Search Documents.
  2. Enter Case #: 201******  (found in Cause Number column of the Active Cause Report).
  3. Enter Case Type (without quotes):  "PUD" or "OSF" (OSF for OUSF related causes).
  4. Press the "Search" button.
