DBE Form 6 and DBE Quote Submittal
49 CFR Part 26.53 allows each agency to decide if DBE quotes will be submitted with bid responses or within five (5) calendar days of bid opening. ODOT is returning to a 5 calendar day submittal. For an ODOT bid opening on a Thursday, the Form 6 and Quotes are due by the following Tuesday by 4:30 pm.

General Process
The Prime Contractor completes its portion of the form, including attaching quotes, listing quote lines, and totaling on page 2, and submits the form to the DBE Subcontractor.
The DBE Subcontractor signs if in agreement.
The form routes to ODOT.
This must be completed within 5 days of bid opening.
Note: If the Prime Contractor receives an email requiring verification of the email address prior to the form being sent to the Subcontractor, please complete this step. It will not route to the DBE without this. We’ve removed the requirement on our administrative side, but it still comes up occasionally. A ticket has been sent to Adobe.
