OHCA strongly believes that SoonerCare providers have the knowledge and desire to deliver quality health care to SoonerCare members. However, we believe that doing so has become an ever increasing challenge, given the demands placed upon all providers.

The SoonerCare HMP is an innovative, comprehensive program which aims to benefit SoonerCare Choice members who have or are at risk for developing chronic conditions as well as the SoonerCare Choice primary care providers who care for them.

Program Goals:

To improve member health outcomes,
To improve quality of care, and
To reduce long-term health care spending

Telligen administers the SoonerCare HMP on behalf of OHCA.

This data is treated as confidential and is stored securely in accordance with applicable law and regulations. OHCA may share this personally identifiable data with its authorized business associates, government agencies with jurisdiction over the OHCA, or as otherwise required or permitted by applicable law.
