Health Coaching
SoonerCare Choice members who have or are at risk for developing chronic conditions are eligible for targeted care management through the SoonerCare Health Management Program (HMP). Once enrolled, members are assigned a dedicated health coach who works directly with them to support improved health outcomes.
Health coaches are registered nurses trained in motivational interviewing and chronic disease management. When embedded in participating practices, they provide the following key services:
- Collaborate with members to develop personalized health behavior change plans.
- Help set short- and long-term goals for chronic disease self-management.
- Address medication adherence for members not meeting treatment goals.
- Deliver education on disease-specific topics and general health skills using evidence-based materials.
Because health coaches work on site at your practice, they are well-positioned to keep you informed about your patients' progress, fostering better care coordination and stronger outcomes.
Provider Involvement Opportunities
In addition to benefiting from embedded health coaching, providers can actively engage in the HMP through:
- Participation in quality improvement initiatives supported by practice facilitation.
- Access to evidence-based educational resources tailored to chronic disease management.
