Take Charge is Oklahoma's Breast and Cervical Cancer Early Detection Program (BCCEDP). The Oklahoma State Department of Health, Chronic Disease Service, provides this program through a grant from the Centers for Disease Control and Prevention (CDC) and state match funding. The mission of Take Charge is to provide women with information and access to screening services to reduce morbidity and mortality from breast and cervical cancer. 

Take Charge Services

Services for women who meet the program guidelines may include:

  • A women's health history
  • Cancer risk history
  • Well-woman assessment
  • Clinical Breast Exam
  • Pap Test and limited screening pelvic exam
  • Education for lowering your cancer risk
  • Free Mammogram
  • Referral and follow-up for women with abnormal results

Do you qualify for Take Charge services?

  • Call toll-free 1 (888) 669-5934 and speak to a patient navigator.
  • For non-partner clinics wanting to refer patients for diagnostic services, please complete the ODH Form 1342 and submit to the Take Charge Program by fax (405) 900-7609 or email.
