Take Charge is Oklahoma's Breast and Cervical Cancer Early Detection Program (BCCEDP). The Oklahoma State Department of Health, Chronic Disease Service, provides this program through a grant from the Centers for Disease Control and Prevention (CDC). The mission of Take Charge is to provide women with information and access to screening services in order to reduce morbidity and mortality from breast and cervical cancer. 

Take Charge Services

Services for women who meet the program guidelines may include:

  • a women's health history
  • cancer risk history
  • well-woman assessment
  • Clinical Breast Exam
  • Pap Test and limited screening pelvic exam
  • education for lowering your cancer risk
  • free Mammogram
  • referral and follow-up for women with abnormal results

Do you qualify for Take Charge services?

  • Call toll free at 1 (888) 669-5934 and speak to a patient navigator.
  • Complete the eligibility form (English version or Spanish Version) and submit it to the Take Charge Program either by fax or mail.

Take Charge Program Overview & Eligibility for 2023

Serving low-income, uninsured, and underinsured Oklahomans.

The mission of Take Charge is to provide Oklahoma residents with information and access to screening services to reduce morbidity and mortality from breast and cervical cancer by facilitating early screening, ensuring prompt diagnosis, and improving access to treatment.

  • Uninsured
  • Underinsured (unmet deductible of $150.00 or more)
  • Cis-female
  • Transgender women (male-to-female) (If they have been on hormone therapy for their reassignment)
  • Transgender men(female-to-male) (who have not undergone a bilateral mastectomy and/or hysterectomy) 
  • Non-legal residents of the United States/Undocumented Individual
  • Those eligible for Medicare but who are not currently enrolled, may receive services until their enrollment into Medicare (Take Charge encourages all individuals eligible for Medicare benefits to enroll in Medicare),

Those individuals under age 50 must be surgically sterilized by hysterotomy or tubal ligation.

Ages 21 to 39*:

  • clinical breast exam
  • pap test 
  • pelvic exam
  • HPV test co-testing (Human Papillomavirus) 

Age 40 to 64*:

  • clinical breast exam
  • screening mammogram
  • pap test
  • pelvic exam
  • HPV test (primary or co-testing available) 

*Eligible patients with signs/symptoms of cancer (breast or cervical) can receive needed diagnostic tests regardless of age.
