HB 3644, The Blake Burgess Act: Effective July 1, 2026, this bill requires hospitals with an emergency department and ambulatory surgery centers to develop and implement policies and procedures to ensure appropriate medical attention for persons at risk of forming venous thromboembolisms, assisting patients at risk, treatment options, and training all non-physician personnel at least annually on policies and procedures developed for this purpose.
SB 1423: Effective Nov. 1, 2026, this bill eliminates the Hospital Advisory Council.
CLIA has transitioned to sending email notifications to laboratories to include electronic CLIA fee coupons and certificates. Paper fee coupons and certificates are no longer available. Fee coupons and links to certificates will be emailed to the email address on file for the laboratory and come from “Center for Clinical Standards and Quality” or from noreply-cms-ccsq@ccsq.hhs.gov.
Fees can be paid online using ACH transactions, debit or credit card
If I have lost or misplaced my CLIA certificate, how can I get a replacement?
You may review your certificate information and print a current CLIA certificate using the CLIA laboratory lookup link
If your current certificate is not available to print, email clia@health.ok.gov for assistance.
When will I receive my CLIA certificate once my payment has been made and received by CMS?
A link to print your new CLIA certificate will be emailed approximately 2-3 weeks prior to your certificate expiration date or within 10-14 days for an initial certificate.
How do I change the laboratory director on my CLIA certificate?
Submit your request to change the director on your CLIA certificate on the CMS-116.
All 5 pages must be filled out completely and signed.
