- Oklahoma licensure of Ambulatory Surgical Centers required by:
- Hospitals
- General Medical Surgical Hospitals; Critical Access Hospitals and other Specialized Hospitals: Psychiatric, Rehabilitation

- CLIA has transitioned to sending email notifications to laboratories to include electronic CLIA fee coupons and certificates. Paper fee coupons and certificates are no longer available. Fee coupons and links to certificates will be emailed to the email address on file for the laboratory and come from “Center for Clinical Standards and Quality” or from noreply-cms-ccsq@ccsq.hhs.gov.

- You may review your certificate information and print a current CLIA certificate using the CLIA laboratory lookup link
- If your current certificate is not available to print, email clia@health.ok.gov for assistance.
- A link to print your new CLIA certificate will be emailed approximately 2-3 weeks prior to your certificate expiration date or within 10-14 days for an initial certificate.
- Submit your request to change the director on your CLIA certificate on the CMS-116.
- All 5 pages must be filled out completely and signed.
- The form can be emailed to CLIA@health.ok.gov or faxed to (405) 900-7559.

Hospital Advisory Council
The Hospital Advisory Council is authorized by statute (63 O.S., Section 1-707) to serve as an advisory body to the Department. The Advisory Council is composed of nine members appointed by the Commissioner with the advice and consent of the Board of Health. The membership of the Advisory Council is as follows: Two members are hospital administrators of licensed hospitals; two members are licensed physicians or practitioners who have current privileges to provide services in hospitals; two members are hospital employees; and three members are citizens representing the public who are not hospital employees, do not hold hospital staff appointments and are not members of hospital governing boards.
Members are appointed for a three year term. The Board must meet at least quarterly and may hold such special meetings as may be necessary.
The Advisory Council has the duty and authority to: (1) review and approve in its advisory capacity rules and standards for hospital licensure; (2) evaluate, review and make recommendations regarding Department licensure activities, provided however, the Advisory Council shall not make recommendations regarding scope of practice for any health care providers or practitioners regulated pursuant to Title 59 of the Oklahoma Statutes, and (3) recommend and approve: quality indicators and data submission requirements for compliance with licensure requirements.
