Service Information

Prader Willi Syndrome (PWS) services is a program designed to support adults. The program is designed to assist the individual in achieving the maximum level of independence possible. In this setting the individual lives in a home with staff support 24 hours per day with specialized programming designed to address the unique challenges associated with the Prader Willi diagnosis.

Individual must rise to the level described in policy OAC 340:100-5-34

  • Individuals 18 years or older
  • Diagnosis of Prader Willi Syndrome (PWS)
  • Must be approved to receive services per OAC 317:40-1-1

  • Services are offered when funding is available, when funding is not available the individual may be added to the PWS statewide waiting list.
  • The waiting list is maintained in chronological order based on the date of receipt of a written request for PWS services.

  1. Name
  2. Contact information
  3. Birthdate
  4. Social Security Number
  5. Name and contact information for advocate
    A copy of the medical testing will be requested at a later date.
