Benefits Applicant

If you do not have an open case with SoonerCare, you need to apply for SoonerCare by selecting ‘Apply Now’.


SOONERCARE MEMBER

SoonerCare Member

If you fall into one of the following categories and have an active SoonerCare case, you are in the SoonerCare program and have eligibility determined through OHS:

  • Age 65 or over
  • Blind (any age)
  • Disabled adults
  • Disabled children who do not qualify for Social Security Income because of their parents' income and/or resources (TEFRA)
  • Individuals who reside in nursing homes, yet qualify for SoonerCare
  • Individuals with Medicare coverage that need assistance to pay premiums, coinsurance and/or deductibles
  • Home and community-based waiver populations.
  • Individuals who receive treatment for Tuberculosis (TB). Benefits through SoonerCare are limited to TB drugs and TB clinic visits and only if the individual has active TB. These benefits are for a limited amount of time only.

If you need to make updates to your case such as income or address changes, contact OHS at 405-522-5050. If you need to ask questions about your benefits, need information about claims, or would like to change your PCP, contact the SoonerCare Helpline at 1-800-987-7767 and speak with Member Services.


American Indian/Alaskan Native (AI/AN)

If you are an American Indian or Alaskan Native you have the option to opt-in to SoonerSelect or remain on SoonerCare. You can log into your member account at mysoonercare.org and see if you are enrolled in SoonerSelect by looking on the benefits page under ‘Plan/Provider.’ If you show any of the following, you are in SoonerSelect:

  • Aetna Better Health
  • Humana
  • Oklahoma Complete Health
  • DentaQuest
  • LIBERTY Dental

If none of these are listed, you are a SoonerCare member.


SOONERSELECT MEMBER

SoonerSelect Member (Coming Soon)

If you fall into one of the following categories and have an active SoonerCare case, you are in the SoonerSelect program. You can log into your member account at mysoonercare.org and see the plan you are enrolled in on the benefits page under ‘Plan/Provider.’

  • Children, including foster children
  • Low-income parents
  • Pregnant women
  • Non-disabled adults ages 19-64

If you fall into one of the following categories and have an active SoonerCare case, you are in the SoonerSelect program and will need to call the SoonerCare helpline at 1-800-987-7767 to find out what plan you are enrolled in.

  • Children involved in juvenile-justice
  • Children receiving adoption assistance.

 

American Indian/Alaskan Native (AI/AN)

If you are an American Indian or Alaskan Native you have the option to opt-in to SoonerSelect or remain on SoonerCare. You can log into your member account at mysoonercare.org and see if you are enrolled in SoonerSelect by looking on the benefits page under ‘Plan/Provider.’ If you show any of the following, you are in SoonerSelect:

HEALTH CARE PROVIDER

Health Care Provider

You are (or work for) a health care provider who is contracted with SoonerCare or would like to become contracted with SoonerCare.
